If you are booking a Shamanic Healing sessions with Hayley, please first fill out the following agreement form.
- I, hereby acknowledge and agree to the terms outlined below, in recognition of my participation in shamanic healing sessions conducted by Hayley Trezise, Shamanic Practitioner.
- Voluntary Participation – I acknowledge that my participation in shamanic healing sessions is voluntary and of my own choosing. I understand that the healing methods employed may involve non-ordinary realities and spiritual practices.
- Complementary Nature – I acknowledge that shamanic healing is complementary to conventional medical and psychological treatments. I understand that it is not a substitute for professional medical care or advice.
- Informed Decision – I affirm that I have been informed of the nature of shamanic healing practices, its potential benefits, and any potential risks. I am choosing to engage in these practices with a clear understanding.
- Physical and Emotional Well-being – I acknowledge that my physical, mental, and emotional well-being is my own responsibility. I understand that the practitioner may provide guidance, but the decisions I make regarding my health and well-being are ultimately my responsibility.
- Integration and Follow-up – I understand that integration work is an essential part of the healing process.
- Confidentiality – I understand that the details of my healing sessions and personal experiences will be kept confidential by the practitioner.
- Cancellation and Payment – I acknowledge that a 48-hour cancellation notice is required for scheduled sessions. I understand that payment is due in advance of the session unless otherwise agreed with Hayley.
- Results and Outcomes – I understand that individual experiences and outcomes from shamanic healing may vary. I do not hold the practitioner responsible for specific results.
- Personal Insights – I acknowledge that any personal insights or revelations gained during healing sessions are for my own self-discovery and growth. I take responsibility for applying these insights in a healthy and constructive manner.
